Nurse practitioner Warren McDermott has joined the primary care team at Gifford, seeing patients at Rochester Health Center and at the Post-Acute Care Clinic in Randolph. In his PACC role, McDermott follows up with patients who have been discharged from the hospital or emergency department to review their care plan and make sure they have what they need.
McDermott most recently provided care with the Department of Veterans Affairs in Burlington. He is a graduate of Whitman College in Walla Walla, Washington, and holds a doctor of nursing practice degree from the University of Vermont.
