Meridith Vaughan, RN, joins Lamoille Home Health & Hospice as long term care director

Meridith Vaughan, RN, of Moretown, VT, joins Lamoille Home Health & Hospice as its new Long Term Care Director. Meridith has served as Program Coordinator at Out and About for the past four years.
Vaughan will manage the program which provides client-centered care for the frail elderly population and for individuals of any age with permanent disabilities. Our long term care services include homemaker assistance, Choices For Care - through state Medicaid funding, long-term care services are available as an alternative to nursing homes, and case management.
Vaughan received her Bachelor of Science in Nursing from Norwich University. She succeeds Bev Perkins who will continue with the Agency as a Case Manager.
Lamoille Home Health & Hospice serves Lamoille County and provides Skilled Nursing Care, Licensed Nursing Assistants, Personal Care Attendants, Physical, Speech & Occupational Therapists, Hospice Volunteers, Certified Case Managers, a Medical Social Worker, and a team of specialists who work with our community partners and others to be certain care is available and is well managed so that everyone has access to exceptional health care services and can remain at home and in the community, independently, with dignity and maximizing their health and quality of life. The agency is Lamoille County’s only nonprofit home health agency and has served the county for over 40 years.
