Cheshire Medical Center/Dartmouth-Hitchcock Keene (CMC/DHK) welcomes Steven Hertford, MD to the Department of Family Medicine. Dr. Hertford most recently worked as an Emergency Room Physician at Brattleboro Memorial Hospital, Brattleboro, VT, and prior to his tenure there, provided primary care, urgent care and emergency care to patients in facilities in Vermont, Maine and Pennsylvania.
Dr. Hertford provides primary care to individuals of all ages, helping patients maintain health and wellness through preventative treatments such as immunizations, cancer screenings and regular check-ups. He also helps people with chronic conditions understand and manage their illnesses to reduce symptoms and prevent serious side effects. As a Family Physician at CMC/DHK, Dr. Hertford works with other medical practitioners and staff members providing care in a Medical Home Team, helping patients become active participants in their health and well-being. The Medical Home Team integrates all aspects of a person's preventive, acute, and chronic care, offering patients comfort, convenience, and optimal health throughout their lifetimes. CMC/DHK implemented this Medical Home model as a component of the healthiest community initiative in order to help prevent and manage chronic conditions
Dr. Hertford is Board Certified in Family Medicine by the American Board of Family Medicine. He served his Family Medicine Residency at The Medical Center at Heritage Valley-Beaver, Beaver, PA, and completed his Doctorate of Medicine at Rutgers Medical School, University of Medicine and Dentistry of New Jersey, New Brunswick, NJ.
Dr. Hertford also holds a Bachelor of Science from Cook College, Rutgers, The State University of New Jersey, New Brunswick, NJ.
