Consumer Assessment of Healthcare Providers and Systems Survey
If you or your child saw a CHC provider between August 2024 and July 2025, you may have received a patient experience survey in the mail. Please take a few minutes to complete it, as your feedback helps us improve care. A second survey will arrive in mid-November if you missed the first. All responses are anonymous and confidential. Thank you!

Our on-call nurse/provider will be available for any urgent needs by calling your CHC location.
Medication Refills
For most prescription refills, the most efficient method is to contact your pharmacy directly and have them submit a refill request to your doctor. The second-best option is to use the patient portal to request your refill. If you need to contact our office directly, please have all the necessary medication information ready to share.
Be sure to ask your provider during office visits for refills on ALL the medications you’ll need before your next visit.
