Participants now have access up to $9,000 for transition-related expenses
Vermont Business Magazine Money Follows the Person (MFP) Vermont announces that as of August 1, 2025, all eligible and enrolled individuals will have access to $9,000 in MFP transition funds. This is an increase from the previous amount of $2,500.
Money Follows the Person (MFP) Vermont is a state initiative that operates through the state’s Choices for Care Long Term Medicaid program and is dedicated to helping eligible individuals transition from institutional settings back into their homes and communities. MFP provides support and services to enhance independence, improve quality of life, and ensure access to community-based care options for older Vermonters and Vermonters with disabilities.
MFP transition funds are designed to cover one-time and short-term expenses related to transitioning to an approved community-based setting and to address unmet needs that enable individuals to remain independent in the community. Funds can be used to help secure housing, access necessary home modifications or assistive technology devices, fill a pantry with food, and a wide variety of other unmet needs. If an individual has been in a skilled nursing facility or acute care hospital for 60 days or more, is returning home on CFC and has housing available, MFP can support a successful discharge. The increase in transition funds will help pave the way for sustainable return to the community.
MFP’s purpose is to ensure that person-centered planning is at the forefront of the discharge planning process. Facilitating successful discharge means assessing a person’s needs prior to returning to the community so that they have access to the referrals, resources and supports they need in order to sustainably remain back in the community. There are tremendous cost-savings as well as proven increases to quality of life for folks when they are able to reside in their community setting of choice, and MFP is proud to continue to demonstrate this impact across Vermont.
