The primary goal of using Root Cause Analysis (RCA) is to analyze problems to identify 1) What happened, 2) How it happened, 3) Why it happened so that actions to prevent recurrence are developed. In its most basic sense, RCA defines the problem, gathers the information, data & evidence, identifies all issues that contributed to the problem, determines root causes and lastly, identifies recommendations for corrective actions that eliminate the recurrence of problems. This training is ideal for Continuous improvement and Quality/ ISO 9001 personnel and anyone whose job it is to solve their company's problems!
