SOUTHWESTERN VERMONT HEALTH CARE HOLDS CONFERENCE ON ACCOUNTABLE COMMUNITIES AND HEALTH CARE REFORM

WHO & WHAT:

Southwestern Vermont Health Care, with program support from the Vermont Health Care Innovation Project, will present a regional health care conference on building accountable communities in this era of health care reform. The media is invited to visit the conference.

The informative program features Mary D. Naylor, PhD, FAAN, RN, the Marian S. Ware Professor in Gerontology and Director of the New Courtland Center for Transitions and Health at the University of Pennsylvania School of Nursing. For the past two decades, Dr. Naylor has led an interdisciplinary program of research designed to improve the quality of care, decrease unnecessary hospitalizations and reduce health care costs for vulnerable community-based older adults. The program has been implemented with great success at SVMC. Plenary presentations will be made by Kevin C. Stone, BA, MBA, Project Specialist for Accountable Care Organization Development and Regional Services at Dartmouth-Hitchcock, and Heidi A. Klein, MSPH, Public Health Director of Planning and Health Care Quality for the Vermont Department of Health.

Conference partners include Vermont Health Care Innovation Project and Dartmouth-Hitchcock. Supporters include the Organization of Nurse Leaders, Rutland Regional Medical Center, Southern Vermont College, and Vermont Tech

The purpose of the conference is to bring together clinicians, quality/safety improvement specialists, executives, community agency and services representatives, and leaders from across northern New England and beyond to share evidence on the development of accountable care communities that support population health.
