Key outcomes include over 93,000 hours of direct service to more than 3,500 clients
Vermont Business Magazine Community Care Network (CCN) today announced its 2025 service results in conjunction with the release of its 2025 Impact Report.
“CCN is pleased to share its 2025 Impact Report which highlights a range of significant service outcomes and areas of progress from CCN in 2025,” said Dick Courcelle, Chief Executive Officer of Community Care Network. “With major reforms to care delivery and accountability, 2025 marked one of the most consequential periods of change in Vermont’s health and human services system in decades, and we are incredibly proud of the professionalism, creativity and compassion of our staff toward ensuring that all those who rely on CCN remained supported with high-quality services, without disruption.”
Among Community Care Network’s 2025 service results and areas of advancement are:
- 93,387 hours of direct service provided to 3,593 clients across the functional service areas of Developmental Services, Adult Mental Health Services, Child and Family Services, Mental Health Crisis Services and Substance Use Treatment and Recovery Services.
- A total of 195,101 client services provided to Vermonters, including 183,299 client services in Rutland County.
- The average crisis response time by CCN-RMHS’ Emergency Services team was 21.5 minutes, and 96% of crisis responses were carried out in 1 hour or less.
- Certification by the Vermont Department of Mental Health as one of the first two Certified Community-Based Integrated Health Centers (CCBHC) in Vermont, an accreditation and accompanying funding model that supports improved integration of mental health, substance use treatment and recovery and primary care services, as well as increased ability to quickly custom-tailor services to identified community needs, all with an overarching objective of delivering rapid access to whole-person care.
- Launch of a new Veteran Care Navigator program designed to provide specialized support to Veterans and active-duty service members and their immediate families, including care coordination around mental health support, substance use treatment, veterans benefits and housing assistance, ensuring timely and coordinated assistance for those who have served our country.
- Implementation of a new Perinatal Peer Support service designed specifically to assist pregnant and postpartum parents experiencing mood or anxiety challenges and to help them adjust to the demands and emotions of early parenthood.
- Rollout of a new Court Peer program, created in collaboration with the local court system, restorative justice, probation and parole and community partners, which is designed to help individuals who are justice-involved connect to treatment, housing and other basic supports, reducing barriers that can lead to repeat court involvement.
- 93 children served by CCN’s Rutland County Head Start program, which also made substantial infrastructure and programming advancements including the creation of a new developmental classroom, completion of an extensive playground redesign and expansion project, and addition of a new art studio.
- 63,353 hours of service provided by CCN’s Senior and Volunteer Program’s AmeriCorps Senior Volunteers, including over 19,000 hours of support to 351 local students by the Foster Grandparent Program, which celebrated 60 years of operation in 2025.
- Receipt of the 2025 Gold Level Governor’s Excellence in Worksite Wellness Award, presented by the Vermont Governor's Council on Physical Fitness and Sports and the Vermont Department of Health.
