VITL partners with Outcome Healthcare to strengthen Vermont’s health data network

A new platform will give Vermont’s providers faster, more complete access to patient information so Vermonters receive informed and coordinated care built around them.

Vermont Business Magazine When you walk into a clinic in Vermont — whether it’s your primary care doctor or an emergency room you’ve never been to before — your providers caring for you should have easily accessible information about you, like medications and medical history. It’s a reasonable expectation, and for more than twenty years, VITL has been the organization working quietly behind the scenes to make it a reality.

Today, VITL, the independent nonprofit that operates Vermont’s statewide health information exchange (HIE), is taking that work further and announcing its migration to a new clinical data repository and provider portal platform powered by Outcome Healthcare. Outcome brings more than 60 years of combined health information exchange expertise, with focus on advancing interoperability and improving patient care. VITL and Outcome Healthcare plan to complete the migration during Spring 2027.

“We are so excited about the opportunity to work with the team at Outcome Healthcare,” said Randy Farmer, President and CEO of VITL. “They have an outstanding track record of being trusted partners. This new relationship will enhance the quality of VITL’s services and positively impact the delivery of healthcare throughout Vermont.”

The upgrade means that Vermont’s providers — from large health systems to independent rural practices, from EMS crews to home health agencies — will have faster, more complete, and more actionable access to the clinical information that helps them care for patients safely and efficiently.

“The team at Outcome Healthcare is excited to empower Vermont’s healthcare community by putting the right information into the right hands,” said Justin Kaltenbach, the company’s CEO. “By eliminating data silos, we ensure that clinicians across Vermont can make faster, more accurate decisions — ultimately leading to safer, more coordinated care for every Vermonter.”

Wherever you seek care in Vermont, VITL travels with you

VITL is Vermont’s independent health data network — the nonprofit infrastructure that connects the people and systems caring for Vermonters, so that better information leads to better care.

VITL’s participant network spans health systems, payers, EMS agencies, ambulatory practices, public health agencies, Federally Qualified Health Centers, home health agencies, and all of Vermont’s hospitals. The Vermont Health Information Exchange (VHIE) holds health records for 1.3 million people and processes 61 million clinical messages every year, which helps providers see a patient’s full health picture.

Founded in 2005, VITL’s only job is to make Vermont’s healthcare system work better for the people who depend on it. Its independence, combined with two decades of deep trust built with organizations of every size across the state, is what makes VITL the right home for Vermont’s health data infrastructure as the state’s healthcare system evolves.

A platform built for what Vermont’s health care system needs now

The new Outcome Healthcare HIE platform provides VITL’s participant network with new capabilities that will directly translate into better experiences for patients and providers alike:

●      No more starting from scratch: Providers get faster access to a more complete, longitudinal view of patient information like medications, history and recent encounters, at the point of care. When a patient walks in for the first time, the clinical information is already there.

●      Care that follows the patient: Improved real-time care coordination means smoother transitions between providers, fewer gaps, and clinical teams that are working from the same information, even across different organizations and systems.

●      Insights that help providers act: Rapid, intuitive analytics dashboards offer clinicians and health systems the data to monitor outcomes, identify populations that need attention, and demonstrate the impact of the care they deliver, including for federal program reporting.

●      Data quality you can count on: A robust data quality pipeline ensures that the information flowing through the network is accurate, consistent, and complete from the moment it enters the system to when it reaches a clinician’s screen.

●      Built for the future: New technology positions Vermont’s health information network for the next generation of interoperability standards, and the ability to evolve with real-world needs.

●      Trusted security: The platform is HIPAA compliant and meets the highest standards for protecting the sensitive health information Vermonters trust VITL to safeguard.

This advancement arrives at a critical moment, as Vermont’s healthcare stakeholders work to transform how care is delivered across the state. VITL’s upgraded platform, powered by Outcome Healthcare, will improve access in rural communities, reduce unnecessary costs, and build a system that works for every Vermonter regardless of where they live.
