Vermont Business Magazine Age Well has earned Case Management Accreditation from the National Committee for Quality Assurance (NCQA), a leading independent organization dedicated to improving health care quality.
Age Well received NCQA’s highest accreditation status effective June 1, 2026. The three-year accreditation extends through June 1, 2029, and recognizes organizations that meet rigorous standards for case management, care coordination, performance measurement, continuous quality improvement, and person-centered service delivery.
The accreditation underscores Age Well’s commitment to delivering compassionate, evidence-based support that helps older Vermonters remain healthy, independent, and connected to their communities.

A Lifeline for Older Vermonters
Age Well helps older adults remain independent, healthy, and connected by providing essential services across Chittenden, Addison, Franklin, and Grand Isle counties.
During the past year, Age Well:
- Provided 318,689 nutritious meals through Meals on Wheels and community dining programs.
- Supported more than 16,000 older adults through care coordination, transportation, evidence-based wellness programs, caregiver support, and other essential services.
- Engaged 1,092 volunteers who contributed more than 69,000 hours helping older Vermonters remain safe and independent at home.
