Community Care Network launches Veteran Care Navigator Program

Vermont Business Magazine Community Care Network-Rutland Mental Health Services (CCN-RMHS) has launched a Veteran Care Navigator program, designed to provide specialized support to Veterans and active-duty service members and their immediate families, ensuring timely and coordinated assistance for those who have served our country. 

CCN-RMHS Veteran Care Navigator services, facilitated by Brodie Johnson, will provide care coordination to individuals around mental health support, substance use treatment, Veterans’ benefits and housing assistance, among other supports. 

“As a state-designated Certified Community-Based integrated Health Center (CCBHC), Community Care Network is committed to providing equitable, accessible and comprehensive care to all members of our community,” said Cindy Taylor-Patch, Chief Quality Officer with Community Care Network. “Veterans, active-duty service members and their loved ones face unique and complex mental health and substance use challenges. The CCN-RMHS Veteran Care Navigator program was strategically developed by professionals who are deeply committed to honoring the service of Veterans and understanding the challenges they face. With compassion and respect, the program helps break down barriers to care and ensures Veterans don’t fall through gaps in the healthcare system.” 

The CCN-RMHS Veteran Care Navigator program is equipped to help individuals and families with a wide range of intricate challenges including the lingering effects of trauma, service-related injuries, difficulty accessing resources, and stress and emotional burden carried by family members and caregivers, both during and after an individual is deployed. 

“Drawing on personal experience as well as professional expertise, Brodie serves as a trusted guide and advocate” said Taylor-Patch. “She fosters hope, trust, empowerment and self-direction as veterans and their families set goals, navigate life stressors and find stability in a supportive, recovery-oriented environment.”
