AHS launches of Vermont HCBS Grant Opportunity

Vermont Business Magazine The Vermont Agency of Human Services (AHS) is pleased to announce the launch of the Vermont Home and Community-Based Services (HCBS) Grant Opportunity, a significant step toward enhancing, expanding, and strengthening Medicaid HCBS services in Vermont.

The Vermont HCBS Grant Opportunity is funded through Section 9817 of the American Rescue Plan Act of 2021 and is aimed at bolstering the quality, accessibility, and sustainability of Vermont’s system of care for individuals and their families who rely on HCBS to lead independent and fulfilling lives. This grant opportunity, totaling $20.9 million, is designed to support HCBS providers, community-based organizations, and other entities that support the HCBS landscape in Vermont. 

Funding is available across four grant tracks that address critical investments in infrastructure, enhance workforce capabilities, drive care model innovation, and strengthen provider processes.

This initiative is driven by AHS’s commitment to creating a more inclusive, person-centered, and sustainable HCBS system in Vermont. The grant application process opens doors for innovative solutions to improve care delivery, workforce stability, and infrastructure. It aims to reduce disparities and ensure equitable access to HCBS.

The grant application opened September 7, 2023, and will remain open until October 12, 2023, at 5:00 pm EDT. We encourage all eligible organizations to participate and take this opportunity to contribute to the betterment of HCBS in Vermont.

What are HCBS?  HCBS includes medical and non-medical services that are provided in a home or community-based setting rather than an institutional setting. A variety of populations, including people of all ages with intellectual or developmental disabilities, physical disabilities, mental health conditions, or substance use treatment needs, receive HCBS to assist with daily living. Examples of HCBS include home health care, personal care services (dressing, bathing etc.), case management, and substance use disorder treatment. HCBS are offered through a variety of programs, including but not limited to the Choices for Care Program, Developmental Disability Services, Brain Injury Program, Community Rehabilitation and Treatment Services, and Mental Health Under 22.
