Vermont Business Magazine Today, the Accountable Care Learning Collaborative (ACLC) released a series of 20 Case Study Briefs (CSBs) as part of its latest initiative to help providers identify and implement the care delivery competencies necessary to succeed under risk-based payment. InSeptember 2016, the ACLC released its first iteration of recommendedcompetenciesalong with a "Call to Collaborative Action," petitioning health care organizations to actively engage in sharing solutions. Today's announcement uses that work as a foundational framework for targeted, tactical learning.
The Vermont case under the HealthIT section for OneCare Vermont is called:Reducing Variations in Care through Transparent Data Reporting.
In part, the OneCare Vermont report states: "Although the report is still new, OneCare has already seen anecdotally that the communities are beginning to change their post-acute referral patterns from skilled nursing facility (SNF) to home health or directly to outpatient PT.The most profound impact has been in one community identified by the report as the leading user of SNFs as a discharge destination. Equipped with the report, the region is now conducting an internal analysis to understand the reason for its higher SNF usage and to educate its physicians on appropriate and costeffective referral practices."
Each CSB describes one provider organization's approach to successfully implementing a single competency, including solutions, tools and vendor partners used, challenges with implementation, and results to date. These concise accounts will give provider leaders the opportunity to learn from the experiences of others in the industry. Over time, the ACLC will develop a robust library of standardized CSBs with a myriad of competency-specific and comparable industry solutions across a variety of provider types, markets, and maturity levels. These documents will be open to the public and updated regularly.
