Adult Family Care Homes to be implemented by Vermont Department of Disabilities, Aging, and Independent Living

The Department of Disabilities, Aging, and Independent Living (DAIL) announced today that the Adult Family Care (AFC) Home program will begin in May. This new option in home and community-based living is now available to elderly Vermonters and adults with physical disabilities who meet Highest/High Needs Choices for Care eligibility. AFC Homes are private homes that are approved to provide continuous individualized supports in an environment that is safe and family-oriented.
The Adult Family Care Homes program will contribute to DAILs mission to make Vermont the best state in which to grow old or to live with a disability with dignity, respect, and independence, said Commissioner Susan Wehry. The programs goal is to offer a new choice to Vermonters who can no longer live alone.
It is an exciting opportunity that we are able to add the Adult Family Care Home option to our Choices for Care program menu, providing a new choice to Vermonters who wish to remain in a home-based setting. said Megan Tierney-Ward, Choices for Care Program Manager. AFC Home participants, who transition from nursing facilities, may also be eligible for additional assistance and funding through the Choices for Care Money Follows the Person Demonstration grant.
About the Choices for Care Long-Term Care Medicaid Program:
Choices for Care (CFC) is a Medicaid-funded, long-term services and supports program that pays for care and support for older Vermonters and adults with physical disabilities. The overall goal of Choices for Care is to give people more choice and control over where and how they want to meet their long-term care needs.
People who need nursing home level of care receive help with everyday activities and personal care. Choices for Care Services can be provided in their own home, in a Residential Care/Assisted Living Home or nursing facility, and now in Adult Family Care Homes.
