The mission of the Community Health Centers of Burlington (CHCB) is to improve the health of all within the communities we serve. We are dedicated to providing exceptional medical, dental and behavioral health care with respect and compassion, regardless of financial status or life circumstance.
For over 40 years, CHCB has been the premier safety net provider in care for vulnerable populations. The breadth and scope of our health care and support services not only makes us one of the largest primary care facilities in the area, but an expert in care for homeless, low-income and refugee populations.
As one of Vermont’s Federally Qualified Health Centers, CHCB cares for 18,000+ patients – a number representative of our compassionate care, array of services, and dedicated, professional staff. We offer convenient, expanded hours, multiple locations, and special support services to encourage patients to take ownership in their own health care.
As a nonprofit organization, CHCB is sustained through many sources. This includes community support from individuals and businesses, local, state and federal money, as well as revenue from operations.
What is an FQHC?
Spread across 50 states and all U.S. territories, more than 1,100 Federally Qualified Health Centers (FQHCs) provide vital primary care to more than 17 million Americans.
Directed by boards with majority consumer membership, FQHCs focus on meeting the basic health care needs of their individual communities. FQHCs, including CHCB, serve all community residents, including people enrolled in employer-based insurances, Medicare/Medicaid, or public health insurances.
Under our umbrella of comprehensive services, patients can access primary and preventative care, including: Medical and Dental Care, Behavioral Health Services, Psychiatry, a Homeless Healthcare Program, Nutrition Services, Substance Abuse Services, OB/Midwifery, Pediatrics, an On-site Laboratory, Insurance Eligibility and Financial Assistance Programs.
CHCB has five locations in Burlington: Our Safe Harbor Health Center located at 184 South Winooski; our School-Based Dental Center located in the Integrated Arts Academy at 6 Archibald Street; our Pearl Street Youth Health Center located at 179 Pearl Street; and our main site, Riverside Health Center, located on 617 Riverside Avenue. Beginning on January 1, 2016, we will open a new site, our South End Health Center, formerly known as Burlington Primary Care, located at 789 Pine Street.
In addition to the sites in Burlington, CHCB has a facility located on the islands, our Keeler Bay Health Center in South Hero.
